Two outer areas, known as\u00a0<strong>pseudoautosomal regions<\/strong>, are shared with the X chromosome. Deeper inside lie two distinct zones: the\u00a0<strong>heterochromatic regions<\/strong>, which provides structural support, and the\u00a0<strong>euchromatic regions<\/strong>, which carries a small section of DNA that is the genetic code necessary for sperm development.<\/p>\n<p>These two internal regions combine to form what\u2019s called the\u00a0<strong>non-recombinant region<\/strong>\u00a0\u2014 or\u00a0<strong>MSY<\/strong>\u00a0\u2014 which contains the\u00a0<strong>AZF region<\/strong>, short for \u201cazoospermia factor.\u201d This region is essential for the normal development of sperm cells.<\/p>\n<p>Within the AZF region, there are three key sections:\u00a0<strong>AZFa, AZFb, and AZFc<\/strong>. The journey of sperm development begins in AZFa, continues in AZFb, and is completed in AZFc. This is particularly common in men with non-obstructive azoospermia or severely compromised sperm parameters.<\/p>\n<p data-start=\"2132\" data-end=\"2239\">For these patients, <strong data-start=\"2152\" data-end=\"2190\">Y chromosome microdeletion testing<\/strong> can offer clarity. It is often recommended when:<\/p>\n<ul>\n<li>A man is planning\u00a0<a href=\"https:\/\/reproart.wpenginepowered.com\/en\/services\/intracytoplasmic-sperm-injection-icsi\/\" target=\"_blank\" rel=\"noopener\"><strong>IVF with ICSI<\/strong><\/a><\/li>\n<li>A previous attempt at\u00a0<strong>micro-TESE<\/strong>\u00a0(surgical sperm extraction) has failed<\/li>\n<li>There is a\u00a0<strong>family history of genetic abnormalities<\/strong><\/li>\n<li>The analysis shows non-obstructive azoospermia (no sperm due to testicular failure, not a blockage)<\/li>\n<li>There are abnormally high levels of follicle-stimulating hormone (FSH)\u2014a sign the testicles may not be producing sperm properly<\/li>\n<li>The testicls are small or underdeveloped without a clear anatomical explanation<\/li>\n<li>Or simply, when all other possible causes have been ruled out<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-77d0a83 e-flex e-con-boxed e-con e-child\" data-id=\"77d0a83\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-1503a2d elementor-widget elementor-widget-heading\" data-id=\"1503a2d\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">What Can the Results Show?<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c15477f elementor-widget elementor-widget-text-editor\" data-id=\"c15477f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>For many, the results of this test can reshape the course of their fertility journey. A\u00a0<strong>negative result<\/strong>\u00a0brings relief \u2014 no deletions were found, and chances for obtaining sperm during microsurgical sperm retrieval remain high. But even if a\u00a0<strong>microdeletion is detected<\/strong>, all hope is not lost.<\/p>\n<p>Some deletions, particularly in the\u00a0<strong>AZFc region<\/strong>, may still allow for successful sperm retrieval through Micro-TESE, with a success rate of up to 80%. On the other hand, deletions in\u00a0<strong>AZFa<\/strong>\u00a0or\u00a0<strong>AZFb<\/strong>\u00a0typically mean that sperm production has stopped at a much earlier stage, making biological parenthood using one\u2019s own sperm highly unlikely.<\/p>\n<p>Further, these genetic deletions may be passed on to male offspring. That\u2019s why, in such cases, patients who proceed with IVF are often advised to undergo\u00a0<a href=\"https:\/\/reproart.wpenginepowered.com\/en\/services\/preimplantation-genetic-testing-for-aneuploidy-pgt-a\/\" target=\"_blank\" rel=\"noopener\"><strong>PGT-A<\/strong>\u00a0\u2014 preimplantation genetic testing for aneuploidy<\/a>.\u00a0 PGT-A may be used to select a\u00a0<strong>female embryo<\/strong>, thereby avoiding the inheritance of the genetic defect on the Y chromosome. At\u00a0<a href=\"https:\/\/reproart.wpenginepowered.com\/en\/locations\/genomics-in-house-reproductive-genetics-center\/\" target=\"_blank\" rel=\"noopener\"><strong>Genomics \u2013 Reproductive Genetics Center<\/strong><\/a>, our in-house lab, we use the\u00a0<strong>PCR method<\/strong>\u00a0\u2014 the gold standard for this type of genetic screening.
